Should carpal tunnel or cubital tunnel syndrome in CMT 1A and HNPP neuropathy be operated on?

Nerve involvement in CMT 1A and HNPP neuropathy (tomacular or pressure palsy neuropathy) may increase the risk of carpal tunnel or cubital tunnel syndrome and complicate their management.

A retrospective study of 112 patients from the Mayo Clinic (USA) showed :

  • that the prevalences of carpal tunnel and cubital tunnel syndromes are increased compared to the general population in HNPP (53.3% and 43.3% respectively) and CMT 1A (11.5% and 5.8%).
  • in HNPP, the operation of the carpal tunnel syndrome was beneficial for 6 out of 10 patients, that of the cubital tunnel only for 1 out of 5, 2 other operated persons having on the contrary worsened.
  • in CMT 1A, 5 out of 6 people operated on for carpal tunnel syndrome experienced an improvement in their symptoms.
